Community Missions’ After Care Specialist, will provide aftercare services through the coordinated entry program to individuals and families newly relocating from the Buchanan County shelter systems/ homeless within the community with rental assistance or entering a city rental assistance program from the community.
Services will respond to the specific needs of families and individuals using a holistic and comprehensive case management service delivery model in order to provide each household with the necessary support from the point of lease signing through post move-in, ensuring a smooth transition to their new communities.
Community Missions’ ACS will provide case management and wrap-around services incorporating referrals to resources and information including legal services and other supports in the community such as substance use treatment programs to prevent preventing reoccurring institutionalization. Community Missions will develop a service plan with the family or individual that incorporates short- and long-term goals and links them to referral services in community. Services may include but are not limited to:
· Communicate clear tenant roles and responsibilities regarding lease, rent and communication with landlord
· Acclimate tenants to their new neighborhood
· Familiarize tenants with nearby transportation, hospitals, schools, daycare, and other services
· Familiarize tenants with community-based resources in neighborhood
· Connect tenants to the Resident Economic Empowerment and Sustainability for economic and educational advancement
· Employment readiness and advancement
· Financial literacy
· Benefits screening
· Legal Services
· Treatment services
· Bus Passes
Qualifications
1. Experience: Experience in working with low-income families through a diagnostic and counseling approach preferred. Ability and willingness to work in a collaborative team environment and promote positive results.
2. Essential Technical/Motor Skills: Computer entry and typing.
3. Interpersonal Skills: Strong communication skills, patience, ability to listen, ability to relate to diverse populations, ability to work with volunteers and donors
4. Essential Physical Requirement: Ability to lift and carry 50 pounds on a daily basis; ability to organize and physically move items and donations. Ability to position self to access lower and upper shelves, cabinets, and files. Ability to ascend and descend stairs.
5. Essential Mental Abilities: Ability to assess client’s problems and potential and make judgement, good knowledge of resources available, organizational skills. Must be able to demonstrate, through case record keeping, appropriate use of social case work methods to achieve results.
6. Essential Sensory Requirements: Ability to communicate effectively, both verbally and through writing case stories of successes achieved.
7. Exposure to Hazards: Some non-invasive client contact, exposure to typical office substances.
Job Types: Full-time, Part-time
Pay: $35,000.00 - $40,000.00 per year
