Duties:

• Supervise and assist the client with ADLs/IADLs as indicated by the care plan 

• Diet planning and feeding assistance 

• Reporting changes in the client’s condition or living environment

• Assisting with ambulation

• Home Management, Home Safety, and Sanitization 

• Routine housekeeping as indicated by the care plan

• Infection Control

• Other Duties as Assigned

Minimum Requirements:

• High School Diploma or Equivalent 

• Must be 18 years or older and able to read and write consistent with job requirements 

• Certificate of formal training preferred 

• Ability to participate in physical Activity 

• Ability to do extensive bending, lifting, and standing on a regular basis

• Heavy Lifting 30lbs. minimum

Professional Skills:

• Ability to prioritize 

• Be flexible, innovative and possess good interpersonal skills 

• Ability to work independently, make sound judgments based on the care plan and data available, and act accordingly
