Integrated Oncology Network is Hiring a Patient Navigator Near Fresno, CA
- Performs assessment of patient care needs for new and ongoing patients.
- Work with physician and clinical office to facilitate, coordinate and ensure patient has access to available resources.
- Serve as an essential link between patients and all other providers. Central point of contact for all treatment patients; including interaction with medical, nursing, ancillary services.
- Collect and report data from patient on barriers and intervene as appropriate for optimal patient care.
- Assist patients with referrals to outside resources and care coordination as needed.
- Follows established departmental policies, procedures and continuous quality improvement objectives.
- Collaborates with medical providers, patient care staff and clinic management in the planning and implementation of patient and staff education.
- Designs or participates in the creation of educational materials.
- Performs miscellaneous job-related duties as assigned.
- High school diploma or equivalent required
- Graduate of an accredited School of Nursing ( RN/LVN ).
- Strong customer service background, preferably in healthcare environment
- Excellent verbal communication skills
- Ability to manage and prioritize multiple tasks.
- Proficiency in computer processing functions and other standard office equipment
- Ability to calmly and professionally resolve customer issues with diplomacy and tact
- Ability to work independently with minimal supervision.
- Strong organizational skills
- This position functions primarily indoors in a medical / business environment. Employee may be subject to blood borne pathogens, radiation, bio waste and other medically-related factors, with exposure to moderate noise levels.
- Visual acuity to read and compute screen and paper documents; close and distance vision, peripheral vision depth perception, ability to adjust focus; hearing acuity to converse with staff and customers;ability to walk, stoop, bend, kneel, and lift and move 25 pounds on a regular basis; ability to sit for hours at a time. Employee will be exposed to moderate noise levels, interruptions, and occasionally asked to work evening and weekends as needed.
