- The Home care Intake & Outreach Liaison is responsible for making outreach calls to patients and their families - to guide & educate them on homecare services options.
- Initiate outreach calls to social workers & hospital discharge planners, in order to coordinate care for patients in need of short-term & possible long-term care services.
- Communicate with health plan care managers & coordinators to follow up on plan of care and service authorizations.
- Follow up with patients and their families to ensure plan of care is being expedited.
- Chinese and/or Spanish speaking is needed to perform the outreach calls.
Qualifications: High school diploma or general education degree (GED) required. Associates degree or certificate/degree from a technical school preferred. Two (2) years experience in a social services or healthcare related program involving State or City programs such as Medicaid or Food Stamps preferred.

Pay: $600.00 - $700.00 per week
