The region’s leading network of healthcare providers, Inspira Health is seeking a Social Worker to join us at Inspira Medical Center Mannington in Salem, New Jersey. We’ll rely on you to meet a variety of social service needs for patients and caregivers as they transition to the appropriate level of care.

Responsibilities:

• Complete psychosocial assessments for specific patient populations including dialysis, postpartum and oncology
• Collaborate with the interdisciplinary team in discharge planning for high-risk patients
• Maintain patients’ records by reviewing case notes and logging events and progress in a timely manner
• Participate in interdisciplinary rounds
• Educate patients and provide them and their caregivers with appropriate community resources and agencies to help ensure safety upon discharge

Qualifications:

• Graduate from an accredited School of Social Work with an MSW degree
• State licensure required by the NJ State board of social work examiners
• Licensed social worker (LSW) or licensed clinical social worker (LCSW) in New Jersey
• Minimum of one year of post-master’s in social work in the healthcare setting
• Excellent writing skills and effective communication skills
