The Rehabilitation Admissions Liaison Team Lead demonstrates strong leadership skills in identifying appropriate patients for the acute rehabilitation setting while supporting the liaison admissions team to do the same. This team lead develops and fosters positive relationships with ongoing and potential referral sources and guides the group to respond timely and appropriately to referrals from multiple sources and modes. Anticipates and monitors changes in market trends and referral patterns. Assists with plans and presentations of program information to referral sources, patients, families, and professionals. Communicates with internal/external contacts in a collaborative style that is timely and meets the needs of referral sources, healthcare team members, patients, and families. Facilitates the rehabilitation admission and referral process to ensure timely patient flow. Maintains a working knowledge of insurance authorization and benefits requirements for rehabilitation services for programs, facilities, and payers. Identifies, and supports operational functions related to the daily oversight of clinical liaison workflows, including reviewing medical records with team members to clarify if patients are ready for transition to acute rehab, assist with operational needs of the department, and provides coverage for care sites/referral sources as needed.

Required Licensure - Must be licensed in 1 of the following in Virginia: Registered Nurse; Physical Therapist; Occupational Therapist; or Speech Language Pathologist Experience - 2 years of clinical experience in an acute rehab, acute care or case management setting
Education - Bachelor's Degree Nursing, Physical, Occupational or Speech Therapy
