Innovative Diabetic Center, a leading healthcare organization dedicated to providing innovative and patient-centered care, is seeking a highly motivated and experienced Nurse Practitioner to lead our newly established Diabetic Infusion Therapy Program. Position Overview:

We are looking for a dynamic and autonomous Nurse Practitioner to take on the role of Program Manager for our Diabetic Infusion Therapy Program. The successful candidate will play a crucial role in overseeing and managing the program, providing direct patient care, developing chronic care plans, and collaborating with referring providers to optimize patient outcomes.

Responsibilities:

Clinical Leadership:
- Provide direct patient care through diabetic infusion therapy.
- Conduct comprehensive assessments, develop treatment plans, and implement evidence-based care practices.
- Monitor and evaluate patient progress, adjusting treatment plans as needed.
Program Management:
- Oversee the day-to-day operations of the Diabetic Infusion Therapy Program.
- Collaborate with interdisciplinary teams to ensure seamless patient care coordination.
- Develop and implement protocols, policies, and procedures to enhance program efficiency and effectiveness.
Chronic Care Planning:
- Develop personalized chronic care plans for patients with diabetes, focusing on long-term management and prevention of complications.
- Educate patients and families on self-management strategies and lifestyle modifications.
Communication and Collaboration:
- Establish and maintain effective communication channels with referring providers to ensure continuity of care.
- Collaborate with healthcare professionals, including physicians, nurses, and other specialists, to optimize patient care.

Qualifications:

Current and unrestricted Nurse Practitioner license in AZ.
Certification as a Nurse Practitioner with a specialization in diabetes management preferred.
Minimum of 1 year of experience in diabetes care or related field.
Strong interpersonal and communication skills.
