Essential Duties
Assists with recruitment and enrollment or participants in home setting.
Provides emotional support to patient and their families, fostering open communication and support
Assists staff in identification and connection to resources for patients and families.
Develops relationships with community organizations and facilities to secure resources and helps to complete appropriate applications.
Develops and maintains a resource list for common goods and services needed.
Collaborates with healthcare team
Assists in coordinating home health services, medical equipment and things related to medication management.
Maintains accurate and up to date documentation of up-to-date documentation of patient interactions and interventions
Ensure compliance with all relevant healthcare regulations and standards
