• Completed TDH approved CHW Certification course
• High school diploma or equivalent
• Excellent observation, communication and relationship building skills
• Proven experience in healthcare or community outreach role.
• Compassionate and empathetic approach to patient care
• Ability to work independently and collaboratively within a team.

• Assists with recruitment and enrollment or participants in home setting.
• Provides emotional support to patient and their families, fostering open communication and support
• Assists staff in identification and connection to resources for patients and families.
• Develops relationships with community organizations and facilities to secure resources and helps to complete appropriate applications.
• Develops and maintains a resource list for common goods and services needed.
• Collaborates with healthcare team
• Assists in coordinating home health services, medical equipment and things related to medication management.
• Maintains accurate and up to date documentation of up-to-date documentation of patient interactions and interventions
• Ensure compliance with all relevant healthcare regulations and standards
