REQUIREMENTS

* Current Registered Dietian credential and Licensed Dietitian/Nutritionst by the state or states im which they practice.

* Experience in Skilled Nursing/Long-Term Care.

* Strong communcation, clinical nutriti9on, and customer service skills

*Problem-solving skills, care coordination, and clinical coordination skills, and maintaining regulatory compliance.

* Strong time management planning, organization, and ability to work independently.

* Must be flexible with traveling to meet on-site support services and requirements..

REGISTERED DIETITIAN/NUTRIONIST JOB RESPONSIBILITIES

* Lead Nutrition Expert for the facility

* Clinical duties, nutrition assessments, care plans, NAR/SWAT, and education

* Active member of the Interdisciplinary Team (IDT).

* Complete Quality Assurance and Qualityh Improvemnt Audits

* Documentation with the Nutrition Care Process in the EMR system.

* Collaborating with the interdisciplinary care plan team.

* Completing the MDS-Section K

* Calculating TF's 

* Making nutrition recommendations

* Completing High-Risk charting

* Participating in the Nutrition-At-Risk Program
