The Transition Advisor will work within assigned hospital accounts meeting with patients, families, and care coordinators within referring hospital networks. The Transition Advisor will work with referral sources from receipt of referral to patient onsite, while working closely with The Admissions Concierge to ensure smooth transition from hospital to health center. As well, the TA will accurately document on CRM, customer and prospect information and maintain good time and territory management to meet the needs of hospital referral volume. The performance of the Transition Advisor will be measured by set goals and conversion % expectations.
