Indiana Joint Replacement Institute is Hiring a Nurse Practitioner - PeriOperative and Care Clinic Near Fort Wayne, IN

Description

Indiana Joint Replacement Institute (“IJRI”) is developing an outpatient perioperative environment which aims to guide patients safely and thoroughly through their surgical procedure. IRJI uses a team-based approach to identify surgical need, optimize medical management, safely provide anesthesia, and perform surgical procedure, with the intent of returning the patient to their home post-operatively to allow for maximal healing. This partnership is in collaboration with Physicians, Nurse Practitioners, Physician Assistants, Nurses, Medical Assistants, Physical Therapist, Occupational Therapists, and others to ensure a smooth transition out of the ambulatory surgical center and back to their own home.

Reporting to the medical director of Peri Operative Medicine, the Nurse Practitioner would leverage progressive and critical thinking skills to provide a comprehensive medical assessment and risk stratification of patients prior to undergoing surgery. The goal of this assessment is to identify comorbid conditions that could lead to complications perioperatively, with the intent of medical optimization, and includes a shared decision-making conversation with patients regarding possible complications related to their chronic medical problems. In appropriate settings, the Nurse Practitioner may recommend referral for further workup and testing prior to planned procedure, with the goal of driving key performance indicators (ex: reduction in cancelled procedures, reduction in 30-day readmissions, reduction in urgent care visits post op, minimizing unneeded referrals and testing, increase patient engagement)
