Moving Home Minnesota (MHM) Transition Case Manager

Department: Moving Home Minnesota (MHM)

Location: 235 E 6th Street, Suite 400A, St. Paul, MN 55101

Job Type: Full-time, M-F, 40 Hours/week

Starting Wage:

$50,000 annual salary for 4 year degree in Social Work/Psychology/Human Services/Sociology or related field

$51,600 annual salary for Licensed Social Workers (LSW)

Wage will increase $2,000 after 6 months of employment

Description: The role of the Moving Home Minnesota (MHM) Transition Case Manager is to assist eligible individuals who reside in eligible institutions, such as nursing facilities, gain access to needed medical, social, educational, financial, housing, and other social services and supports necessary to move from an institution to their own home in the community. Case Managers will collaborate with various counties and managed care organizations (MCO) to help individuals gain access to services and supports. Case Managers will travel to different facilities around the state of Minnesota and will work with approved individuals diagnosed with mental, physical, developmental disabilities, and/or substance use disorders. Case Managers will assist individuals in developing a person-centered transition plan to relocate to the community. Case Managers will implement the transition plan and may purchase items for the individual that is necessary to relocate back to the community. Case Managers will be responsible for determining prospective housing options based on the individual's preferences and budget, removing any barriers that limit housing choices, and connecting individuals with funding sources such as charity grants or government programs. Case Managers will also advocate for the individuals with prospective landlords, assist individuals in applying for housing, and provide support once housing has been found, if requested. The position requires critical thinking and problem-solving skills. As a MHM Transition Case Manager, you will hold a caseload of 5-10 clients, with caseloads varying based on need/difficulty and location. Case Managers will meet with the Administrator on a weekly basis to discuss updates and to consult on caseloads. Case Managers will need to be able and comfortable driving clients to housing tours, etc.

In this position, you will:

  • Hold a caseload of 5-10 clients, with caseloads varying based on need/difficulty and location
  • Travel to conduct a visit with the client and others identified as necessary to develop and implement goals of the plan
  • Coordinate and/or transport clients to/from potential site tours
  • Assist clients in completing housing applications
  • Assist clients in locating suitable housing in the community
  • Develop and implement an individualized person-centered transition plan
  • Coordinate and monitor the implementation of the plan
  • Communicate with the client, legal representative (if applicable), informal supports, service providers and other necessary supports to implement the plan
  • Coordinate referrals and assist in accessing services and housing
  • Locate and coordinate with different types of housing such as, Assisted Livings, Independent Apartments, Market Rate/subsidized apartments
  • Conduct a risk assessment and risk mitigation plan for living in the community
  • Coordinate and plan for discharges from eligible institutions
  • Coordinate meeting(s), medical follow-up appointments, delivery of medical equipment, availability of medications, etc.
  • Assist individuals with obtaining monthly income through Social Security or General Assistance
  • Assist individuals with certification of disability through the State Medical Review Team (SMRT) and Social Security
  • Complete CAF to apply for Emergency Assistance, Cash, Food Support, Utilities, etc.
  • Coordinate with lead agency to set up services for individuals relocating back to the community and arrange details regarding waivered services, PCA, PT/OT, etc.
  • Research and obtain funding for security deposits, moving expenses, furniture, and household items for when an individual transitions to the community
  • Complete and maintain documentation in case notes that supports and verifies activities
  • Perform outreach to nursing facilities and other appropriate agencies regarding MHM
  • Complete paperwork per DHS & ILP’s guidelines
  • Assist with monitoring, sorting, and replying to emails in RSC/MHM referral email inbox
  • Send bi-weekly/monthly updates to lead agencies
  • Attend DHS trainings/office hours quarterly

Expectations:

  • You will work Monday through Friday, starting your shift anytime between the hours of 7am-9am and ending your shift between the hours of 3pm-5pm, working 8 hours each day.
  • You will meet weekly with your supervisor to discuss your schedule, updates, case consults, trainings, etc.
  • Follow appropriate chain of command by discussing issues, questions, and time off requests with your direct supervisor
  • Any time off will need to be discussed with your direct supervisor first for approval, and so that tasks can be divided and covered accordingly during your absence
  • The first 90 days of the position will be a probationary period. At the end of the 90 days, you will receive an evaluation of progress. After this meeting, you will be evaluated on an annual basis from your initial anniversary date to determine how you are doing, what can improve, strengths, goals, etc. Evaluations will include self and peer/staff evaluations. Raises and bonuses will be discussed during annual evaluations and are determined based on your performance.
  • You will complete trainings on Vulnerable Adults and Mandated Reporting, Person-Centered Planning, HIPAA, housing, and trainings specific to RSC/MHM within your first two weeks of employment. Once trainings are completed during your first two weeks, trainings will also need to be completed on an annual basis following

Required Qualifications:

  • Must be at least 18 years or older with a bachelor’s degree in social work, psychology, sociology, human services, or related field
  • Proof of Valid Driver’s license and car insurance
  • Must pass criminal background check administered by the Department of Human Services (DHS)
  • Knowledge of Microsoft Office, including, Outlook, Word, Excel
  • Ability to work well with a diverse population, including people with low-income, who struggle with substance use, have criminal backgrounds, and people with developmental, mental, and physical disabilities
  • Willing to travel long distances to meet clients and tour potential relocation sites
  • Ability to organize and prioritize workload in an efficient manner
  • Strong communication and interpersonal skills
  • Knowledge of social service programs and operations
  • Ability to work independently and follow directions and guidelines
  • A team player and leader who is dependable with a strong work ethic
  • Ability to manage time effectively
  • Demonstrates professionalism in the workplace and in the community

Preferred Qualifications:

  • Case Management experience
  • Experience working with individuals on searching, obtaining, and sustaining housing
  • Knowledge of subsidized/public housing and Section 8
  • Knowledge of public programs, such as MSA Housing Assistance and Housing Support
