Job Title: Peer Support & Youth Transition Coordinator

Job Summary:

Coordinate 1:1 Peer Visitor Program and Youth Transition Programming. Communicate with individuals with disabilities about ILC services and provide IL services to assist individuals to live independently. 

Essential Duties and Responsibilities

Assess the general appropriateness of applicants to provide peer support. 

Provide initial Training to participants who wish to become Peer Visitors.

Provide support, ongoing training, and oversight to Peer Visitors.

Communicate with Peer Visitors at least monthly.

Document volunteer hours spent by Peer Visitors each month.

Get to know Peer Visitors in order to facilitate appropriate matches with participants requesting support (referrals).

Communicate with ILS regarding referrals to help ensure appropriate matches are made. 

Keep a list of referrals and Peer Visitors.

Facilitate appropriate matches between Peer Visitors and Referrals.

Apply for funding from DHS for Summer Transition Programming, as available. 

Complete required paperwork by DHS for Summer Transition Programs – before, during, and after programming.

Plan and coordinate scheduling of curriculum, speakers, activities, tours, etc. for Summer Transition Programs along with IL staff at each office.

Develop flyers and disseminate advertisements to elicit students for Summer Transition Programs.

Work with ILS in each office to manage Summer Transition Applicants and verify eligibility for program.

Inform students if they have or have not been chosen to attend or are on the waitlist to attend, as applicable.

Facilitate program with the assistance of ILS at each office.

Provide outreach and presentations on programming and ILC services.

Develop additional youth transition programs as appropriate in various offices and/or virtually.

Implement outreach and programming for youth, with additional focus on Post-Secondary Youth.

Essential Functions as listed in Independent Living Specialist Job Description 

Qualifications:

Graduation from a four-year degree program in an accredited college or university with a bachelor’s degree in counseling, social work, psychology, human services, or another relevant field. 

Must have the ability to effectively communicate both verbally and in writing. Must be able to communicate with a wide variety of people in a clear, effective, and respectful way. Adhere to etiquette of communicating with and about people with disabilities; including but not limited to using person-first language. Remain positive and understanding of coworker’s differences. Embrace and champion diversity in the workplace. Practice active listening. Remain respectful and appropriate in all actions.
