Job Summary
The Direct Support Professional (DSP) provides support and assistance according to the individual needs of clients receiving services from the Community Support Services Program as prescribed in his/her Individual Services/Habilitation Plan. They provide companionship, guidance, and assistance in activities of daily living to one or more people within the Community Support Services Program. The Direct Support Professional will receive a pre-assigned schedule. It is essential that they demonstrate professionalism, a respect for confidentiality and a commitment to the agency's mission.
Essential Duties and Responsibilities:
a. Supervise client’s activities to ensure their health and safety.
b. Track client’s progress towards their goals stated in the Individual Service Plan.
c. Monitor client’s general condition, appearance, and relay any concerns to the appropriate Staff Supervisor.
a. Log detailed habilitation activities/daily outcomes.
b. Record medications taken by the client.
c. Report toileting or behavioral statistics, as required.
d. Record the client’s health condition daily and report this data in a timely manner.
e. Maintain client confidentiality.
a. Provide assistance in these areas as specified by Individual Service Plan:
· Meal planning, preparation, and cooking.
· Cleaning
· Shopping and Money Management
b. Ensure client maintains good hygiene and provide assistance when necessary with:
· Toileting
· Bathing
· Dressing
· Dental Care
· Grooming
· Other necessary hygiene
c. Ensure medication is taken and/or given as prescribed.

6. Essential physical and mental requirements to perform the duties of this position include:
Must be able to work in a constant state of alertness and concentrate for long periods of time to ensure client safety.
Regularly required to talk and/or hear to communicate effectively and accurately with varied individuals and groups. The employee frequently is required to use hands or fingers; handle or feel objects, tools, or controls, write or type information that must be documented. The employee is regularly required to stand; walk; sit; reach with hands and arms; and stoop, kneel, crouch. The employee must often lift and/or assist with physical transfers up to 60 pounds unassisted, 120 pounds assisted. Specific vision abilities required by this position include close vision, distance vision. Occasionally work more than 40 hours per week.
10. Meet the agency’s requirements for drug screening.
Days/Hours: DSPs provide service to clients 24/7, 365 days per year. Individual employee schedule will vary based on the staffing needs and availability.
