Job Description:
We are seeking a highly motivated and experienced Licensed Practical Nurse (LPN) to join our team in Jacksonville, NC. The LPN will be responsible for enrolling patients in our Chronic Care Management and Remote Patient Monitoring programs, and providing ongoing care management and support to patients with chronic conditions. The LPN will work closely with our provider team, clinical pharmacists, and fellow pharmacist to ensure the highest level of patient care.
Job Duties and Tasks:
- Enroll patients in Chronic Care Management and Remote Patient Monitoring programs.
- Provide ongoing care management and support to patients with chronic conditions, including medication management and education.
- Conduct regular check-ins with patients via phone or video calls.
- Monitor patients' health status and alert providers to any changes or concerns.
- Coordinate with clinical pharmacists and fellow pharmacist to ensure patients receive optimal medication management and education.
- Maintain accurate and up-to-date patient records in our electronic health record system.
- Participate in team meetings and collaborate with other healthcare professionals to ensure high-quality patient care.
- Adhere to all state and federal regulations related to healthcare and patient privacy.
- Active LPN license in the state of North Carolina.
- At least 2 years of experience in a clinical setting, preferably in chronic care management or remote patient monitoring.
- Strong communication and interpersonal skills, with the ability to build rapport and trust with patients.
- Knowledge of chronic care management best practices, medication management, and patient education.
- Experience with electronic health record systems.
- Ability to work independently as well as in a team environment.
