Our comprehensive approach integrates biometric monitoring, in-home nursing, virtual visits, and a reliable supply chain, prioritizing quality and safety.
We are hiring multiple, part-time (.7 FTE) Clinical Liaisons to serve as the face of Inbound Health’s Hospital-at-Home program and liaison between IH and hospital partners and patients. Provides patient care coordination for Hospital-at-Home to ensure continuity of care for program patients. Helps to identify qualified patients for Hospital-at-Home and CMS waiver program. Lead and coordinate patient care activities. Work collaboratively with internal and external partners/stakeholders to promote and cultivate the program.
