Inbound Health is Hiring a Clinical Liaison (Part-Time) Near Saint Paul, MN

At Inbound Health, we provide an enablement platform with full-stack capabilities and proprietary technology and analytics tools customized for hospital and skilled nursing at home. Our flexible partnership structure helps customers leverage their existing assets while filling the necessary gaps for program scalability. With over 6,000 patients served across 350 primary diagnoses since May 2020, we enable our clients to deliver safe, coordinated, and patient-centric care. Our comprehensive approach integrates biometric monitoring, in-home nursing, virtual visits, and a reliable supply chain, prioritizing quality and safety.

We are hiring multiple, part-time (.7 FTE) Clinical Liaisons to serve as the face of Inbound Health’s Hospital-at-Home program and liaison between IH and hospital partners and patients. Provides patient care coordination for Hospital-at-Home to ensure continuity of care for program patients. Helps to identify qualified patients for Hospital-at-Home and CMS waiver program. Lead and coordinate patient care activities. Work collaboratively with internal and external partners/stakeholders to promote and cultivate the program.

RESPONSIBILITIES:

Drive Growth Objectives

  • Assist in the developing an outreach strategy
  • Create and implement plans to achieve business goals
  • Identify new opportunities to increase utilization
  • Identify patients who require IP level of care in accordance with Milliman or InterQual criteria

Develop Clinical Recommendations

  • Review and assess the eligibility of patients in preparation for hospital discharge huddle
  • Make recommendations to discharge planning team and hospitalists for qualifying patients

Discharge Planning

  • Assess prospective patients for eligibility
  • Facilitate/coordinate transition from acute care setting
  • Prepare patient for discharge by assisting with orders patients' clinical needs, and gathering clinical information
  • Ensure all incoming patients are appropriate for the program

UM Function

  • Performing Utilization Management functions, if needed, on behalf of the customer

Education

  • Cultivate and maintain relationships at target sites to educate on program and eligibility criteria for potential patients
  • Collaborating with respective site personnel regarding referrals, processes, and resources

Documentation

  • Completing required paperwork, managing correspondence, and maintaining data in database for continuous improvement opportunities
  • Documentation in CRM platform
  • Other duties as assigned

Expectations

  • On-site presence at respective customer location (Hybrid)
  • Participate in weekend and holiday schedule as needed

REQUIREMENTS:

  • Bachelor’s or Master’s Degree in Nursing
  • 2 years of nursing /direct care experience in a healthcare setting
  • Registered Nurse
  • Experience in marketing / program coordination / program management / public health / home health
  • Knowledge of Stark Laws
  • Strong computer skills including SharePoint, Excel, EMR (electronic medical record)
  • Strong (verbal & written) communication and interpersonal skills
  • Excellent organizational skills
  • Active clinical license
  • Valid Driver’s License
  • Excessive sitting; heavy computer usage; driving
  • Hours include rotating holidays & weekends as needed

Hours: 9:00am-4:30pm CST

Pay: $50,407.00 - $66,496.00 per year

Expected hours: 7.5 – 8 per week
