Overview:
We're seeking a skilled Nurse Practitioner with expertise in wound care for home health services. You'll assess, treat, and manage complex wounds in patients' homes, collaborating with a team and educating patients and families on wound care.
Responsibilities:
1. Assess and treat acute and chronic wounds at patients' homes.
2. Develop personalized wound care plans based on assessments and best practices.
3. Perform wound debridement, dressing changes, and advanced wound care procedures.
4. Monitor wound healing progress and adjust treatment plans accordingly.
5. Collaborate with healthcare team members for coordinated patient care.
6. Educate patients and families on wound care techniques and prevention.
7. Document patient care accurately and adhere to professional standards.
Qualifications:
1. Current Nurse Practitioner licensure with wound care specialization.
2. Wound care certification preferred.
3. NP experience with wound care focus.
4. Proficiency in wound assessment and treatment techniques.
5. Strong communication and interpersonal skills.
6. Ability to work autonomously and with a team.
7. Valid driver's license and reliable transportation.
Job Type: Full-time
Pay: $115,000.00 - $130,000.00 per year
