Iliuliuk Family and Health Services, Inc. is Hiring an Assistant Case Manager Near Unalaska, AK

We are looking for an Assistant Case Manager to increase access to healthcare and improve health outcomes for our patients!

  • Education/Licensure: High school diploma or equivalent required. College degree preferred. BLS required within 1 year of hire.

  • Key Working Relationships: Practitioners, Medical Support Staff, Behavioral Health Clinicians, and Administration
  • Primary Customer Groups: External Practitioners, Patients and their family members, and Vendors/Customers

  • Location: Dutch Harbor Unalaska, Alaska
  • Schedule: Weekdays, Monday through Friday

JOB SUMMARY

Overview: The IFHS Assistant Case Manager provides support for enabling services to patients of the health center. These non-clinical services aim to increase access to healthcare and improve health outcomes and include services such as health education, interpretation, and care coordination activities.

Responsibilities

After completion of orientation and onboarding activities, the IFHS Assistant Case Manager will support the following responsibilities:

  • Supporting Patient Care
    • Supporting any necessary outpatient appointments, with referral follow-up and documentation of progress.
    • Managing Referral Work Queue (WQ) – engage with tracking/updates on Epic WQ and spreadsheet on IFHS shared drive (Waitlist).
    • Arranging for Medicaid travel logistics.
    • Tracking of urgent patient medical transport off-island – includes update of spreadsheet on IFHS shared drive (Waitlist) for both air ambulance and commercial transportation needs.
    • Entering Data for Ages & Stages Questionnaires (ASQ) on website, then scanning in to Electronic Health Record (EHR) and mailing results to parents.
    • Participating in care management meetings with nursing and providers.
  • Supporting Patient Education and Outreach
    • Providing outreach for appointment and screening reminders as identified by care team.
    • Assisting patients in coordinating with IFHS Front Desk for support with completing sliding fee discount application, and for patient education on cost of service(s), insurance coverage, and payment options.
    • Supporting Medicaid/WIC applications.
    • Supporting patients seeking additional financial resources for health needs
    • Preparing patient support and education materials, including pamphlets/booklets for Annuals, Prenatal visits, etc.
  • Documentation Management
    • Receiving and processing invoices for Medical Record requests.
    • Scanning documents into patient charts (monitor for new/updated critical patient documentation as needed).
      • Sorting documents from nurses scan file and scan to EHR.
      • Check for EKG and DOTs every morning, scanning documents on EHR.
  • Other department duties
    • Coordinating patient calls, emails and questions.
    • Participating in staff meetings, department trainings, and community outreach events.
    • Other duties as assigned by the Case Manager.

License/Certification: BLS Certification- Required/ or ability to obtain within 1 year of hire

Education: High school diploma or equivalent required. College degree preferred in health, psychology or social work-related fields.

Experience: No previous experience required. Other healthcare experience is a plus.

ESSENTIAL FUNCTIONS

  • Hearing: within normal limits with or without use of corrective devices
  • Visions: adequate to read 12-point font type with or without corrective lenses
  • Must be able to effectively communicate in English, both verbally and in writing with staff, patients and the general public
  • Must have manual dexterity of hands/ fingers for frequent writing, computer work, filing and preforming/assisting with procedures
  • Must be able to occasionally lift, push or pull up to 50lbs
  • Must be able to subject to prolonged sitting, standing or walking
