Day-to-day Responsibilities:

Call patients to support with scheduling medical appointments. Coordinate with local healthcare professionals to confirm appropriate process for patient and potentially identify capacity to see patient within the Military Health System. Liaise with Health Net staff to troublesheet issues with referral authorizations.

Expected Deliverables: Several (~12 ) daily calls to patients to help coordinate their specialty care. After each interaction with patient, complete Questionnaire containing questions on patient's engagement
