Job Description

The Medical Case Manager will provide medical and non-medical case management services to persons with HIV. Key responsibilities include: 

• Evaluate and enroll patients living with HIV into Ryan White Part B medical case management services
• Assist patients in securing prescription coverage and transportation for medical services
• Coordinate services with other medical case managers
• Participate in HIV quality improvement activities
• Manage minor financial aspects of the grant
• Document activities, outcomes and other ancillary data in a central database

Minimum Qualifications

• Associates degree
• Valid driver’s license
• Able to work four days per week 
  

Preferred Qualifications

• Medical degree (i.e. nursing, medical assistant), social work degree (i.e. LSW, MSW) or other allied health science degree
• Experience working with persons with HIV, persons with substance use disorder, and/or transgender persons
• Knowledge of Microsoft Word and Excel
• Strong communication and interpersonal skills
