St. Luke's Health Partners Team engages with the value-based patient population to provide patient centered care management utilizing best practices to achieve better health outcomes, lower total cost of care, and improve patient experience. The team consists of registered nurses, social workers, pharmacist, registered dieticians, certified diabetic educators, respiratory therapist, community health workers, and outreach coordinators working together to support patients.
