Hypertension Clinic PC is Hiring a Dietitian Near Orange, NJ

Job Description
This position provides nutritional services to patients in the nephrology division, which includes counseling, nutritional assessments, prescriptions, and evaluation of nutrition-related lab parameters, as part of a member of the interdisciplinary team to improve results and maximize outcomes.
Summary of Job Functions
1. Obtains and evaluates nutrition histories of the patients regarding food habits. Assesses patient's knowledge of diet and kidney disease and provides appropriate detailed education to the patient and their family regarding nutritional status, adequacy measures, fluid status, and all nutrition-related lab parameters pertaining to their disease process.
2. Completes written monthly nutritional assessments as well as comprehensive patient assessments for the patient's written care plan in accordance with established Federal and State licensure guidelines. Participates in weekly care plan meetings.
3. Monitors patient's adherence and response to nutrition therapy, and adjusts therapy as needed to achieve and sustain an effective nutritional status. Documents patient's response to therapy in monthly progress notes.
4. Collaborates with a multi-disciplinary team on the management of bone mineral metabolism, nutritional status, and fluid status. Participates in daily Quality Improvement team meetings.
5. Communicates with physicians and facility staff regarding lab result findings. Rounds with physicians in the dialysis center and discusses the patient's lab result findings.
6. Must have good communication and analytical skills and ability to work with professional and non-professional personnel.

Minimum Education & Experience:
Registered Dietitian in the State of New Jersey is required. Licensed Dietitian Member of the American Dietetic Association is preferred. Experience in the adult clinical area is preferred, or at least 2 years work experience in adult clinical dietetics. Dialysis Experience is preferred.
Schedule:
8-10 hour shift, 1-3 days per week
Ability to commute/relocate:
West Orange, NJ 07052. Reliably commute or planning to relocate before starting work (Required)
License/Certification:
Registered Dietitian License Location: In person and Virtual options available
