Retrieves charts from nursing units as needed. Accounts for all discharges and searches for missing charts. Preps and scans medical records and performs QC and Validation on scanned records to assure highest quality of legibility and accuracy. Interviews patients for birth certificate completion according to Center for Vital Statistics. Initiates fetal death certificates and death certificate completion for all hospital deaths according to Center for Vital Statistics guidelines. Indexes and scans paper medical records into the electronic health record, after removing documents that are not part of the permanent record. Prepares medical records for shipment to secure storage facility. Purges permanent records in accordance with state and hospital regulations. Assigns documents for electronic signature. Analyzes charts for physician deficiencies and reports to HIM Department Supervisor. Performs other duties as assigned by supervisor
Education Required High School Diploma or GED required.
Minimum of two-year clerical experience required. Medical Records or medical office experience preferred with experience with an EMR.
