The LCSW under the direction of the Director of Ambulatory Services, the Social Worker provides direct service to clients and their families within the community. Implements and maintains social work schedule, on-call schedule, in-services for the department, assists in staff evaluations, project management, program development. 

1. Active participation in IPOC (Interdisciplinary rounds) working closely with Case Manager, Nurses, and Physicians.
2. Prepares psychosocial evaluations and care plans age appropriate to the patient/family needs.
3. Assesses and diagnoses the psychosocial needs of the patients, caregivers, and families through initial history including medical, psychiatric, substance abuse, community resources, and ADLs. As well as considers risk management and how these needs are impeding the plan of care.
4. Develops treatment plans and initiates clinical intervention utilizing individual and group treatment modalities. Makes all appropriate referrals to other community service agencies in a timely manner, as evidenced by documentation.
5. Demonstrates the skills and judgment necessary to implement the plan of care, interventions, and procedures necessary, with an emphasis on self-determination of the client system.
6. Utilizes clinical skills including individual, couples, and group modalities to better meet the psychosocial needs of the patient and family to develop improved clinical skills and expertise, as demonstrated in weekly/biweekly meetings with the director of social services/program manager.
7. Maintains an up-to-date, accurate and appropriate record of all social work activities in patient’s medical chart.

Requirements:

• Must be a licensed clinical Social Worker 
• Bilingual Spanish/English a Plus.
• Bachelors, master’s degree preferred.
