Reporting to the Director, Community Integration Services, the Program Coordinator is responsible for assisting with the development or implementation of the client’s Person-Centered Plan. This position will supervise a close-knit team, providing direct support to a diverse group of individuals with disabilities, and will be responsible for assuring continuity of care for clients and performing related duties such as providing an individual assistance in procuring entitlements, obtaining and maintaining employment, building a positive support system, and ultimately addressing the needs of each individual and implementing the best plan of action for how we can improve their quality of life.
