$34-$42/hr. (DOE) Work schedule: Primarily daytime hours with some variability based on Admissions
A day in the life may include, but not necessarily limited to:
Working with hospital case managers, the patient, and the patient’ s family to provide placement for skilled nursing services or skilled rehabilitation therapy.
Communicating with the appropriate interdisciplinary team members (IDT) to ensure a smooth transition from a home or healthcare setting to a HumanGood Skilled Nursing Health Center.
Communicating the patient’s plan of care to IDT to ensure consistent quality care.
Monitoring reviews and managing the inquiry process through the Referral Manager System as needed to assist the admissions team.
Supporting the admissions counselor with inquiries from families, hospitals, assisted living, and other referral sources.
Maintaining communication and rapport with hospital accounts, accountable care organizations (ACOs), and other referral sources.
Ensuring utilization of standard admission process via HER system; manages inbound continuity of Care Documents (CCD) via Care quality HER integration.
Communicating the new admission’s specific needs to IDT, primary care physician, or other providers to ensure Person-Centered quality care is provided and to support discharge goals.
Providing Other Related Nursing Duties And Responsibilities As Needed.To be successful in this role you would have:
Current RN license in the state for which applying
Current CPR certification
Prior nursing experience in senior care, skilled nursing, admissions, post-acute or sub-acute care is preferred
