Schedule: Day shift-Monday-Friday / NO on call, Holidays or weekends /Create your own schedule

Ability To Commute/relocate
- Grand Junction, CO: Reliably commute or planning to relocate before starting work (Required)
- Physical Therapist License (Required)
- Provides services in Physical Therapy support in accordance with the physician's plan of treatment. Accepts only clients referred by the Administrator/DON
- Preform as case manager of single discipline therapy patients
- Assists the physician in assessing the client's level of functioning and development (and revision, as necessary) of the plan of care by applying appropriate tests for Physical Therapy
- Records and reports the client's reaction to treatment and any changes in the client's condition to the physician, Administrator/DON and/or case manager
- Instructs and advises the client, family, and other health team personnel in the phases of occupational therapy in which they may assist the client
- Recommends additional care and treatment when necessary
- Participates in once weekly case conference or in-service
- Conducts LPN and CNA supervisory visits as assigned
- Bachelor's degree required
- Minimum of one year of experience as Physical Therapist
- Home health experience preferred.
- Must be licensed as an PT in the State of Colorado
- Must have reliable transportation and company-required auto liability insurance.
- Willing to travel within 20 miles radius.
- CPR Certified.
Salary: $75.00 - $100.00 per hour
