Hui Malama Ola Na Oiwi is Hiring a Licensed Clinical Social Worker Near Hilo, HI

Hui Mālama Ola Nā ʻŌiwi is recruiting is seeking a Licensed Clinical Social Worker (LCSW) to suport the behavioral health needs of Hawaii residents. The LCSW provides direct clinical care for individuals, youth, families and groups, and provides other social services for clients and patients in medical and community settings throughout the Big Island. Provides assistance in programmatic support to the agency and performs other duties as assigned.

Duties and Responsibilities

  • Responsible for issues related to Behavioral Health, Social Determinants of Health and provides related clinical and operational support.
  • Responsible for ensuring best practices are offered to clients/patients. Knowledge & practice of Native Hawaiian and Pacific Island cultures.
  • Conduct and document a comprehensive assessment of the member's complex health, psych, social needs, including health literacy and deficits that identifies high risks and red flags.
  • Gather clinical information which includes past medical history, physical/psychosocial factors, cultural influences, evaluation of health care barriers to include: available support systems, available benefits, community resources, financial, employment, housing, educational, relationships and health information as appropriate to develop and create an effective care plan and interventions.
  • Leverage self-reported information, claims data, and health records to complete the assessment.
  • Analyze psychological and social aspects of individual interrelationships to gain an understanding of an individual's thoughts, feelings, and behaviors. Utilization of evidenced based interventions & treatment for treatment of Anxiety, Depression, Substance Abuse, and other mental health concerns.
  • Assist with referrals to services and other community resources as needed.
  • Collaboratively establish plan of care with the patient, physician, and/or other healthcare professionals taking into consideration the patient's/client's readiness to change.
  • Provide clinical therapeutic services with the utilization of best practices and evidence-based interventions to address emotional and behavioral problems and improve quality of life.
  • Advocate for clients by reaching out to Primary Care Provider (PCP), specialists, hospital, interdepartmental staff, local Mental Health services, schools and community agencies in order to maximize program participation and outcomes.
  • Represent the company in community meetings and confidential multidisciplinary task forces concerning health issues or the provision of health-related services.
  • Perform other duties as assigned.

Knowledge and Abilities Required

  • Thorough knowledge of the principles and practices of clinical research and assessment; Working knowledge of the array of mental health, social and substance use disorder services.
  • Ability to utilize evidence-based practice guidelines in the evaluation and management of performance for utilization management, care management, and transitional care activities.
  • Ability to foster effective and collaborative working relationships and able to recognize and effectively utilize the most appropriate mode of communication-based upon the circumstances of the situation.
  • Ability to utilize good judgment and tact when interacting with health care providers, members and other stakeholders.
  • Ability to work independently, manage assigned workload, make decisions related to areas of functional responsibility, and recognize issues requiring escalation.
  • Ability to identify issues, conduct research, gather and analyze information, reach logical and sound conclusions, and make recommendations for action.
  • Ability to effectively, clearly and independently document, summarize and resolve complex issues.
  • Ability to analyze data and prepare written and oral reports.
  • Ability to develop training materials, in conjunction with the Behavioral Health Department/Director, and conduct trainings.
  • Ability to create forms, correspondence, and other program documents.
  • Knowledge of behavioral health, substance abuse, mental issues, trauma and available services.
  • Good communication skills and demonstrate the ability to develop instant rapport with people.
  • Ability to maintain the confidentiality of client information in accordance with HIPAA compliance.
  • Demonstrated ability to work well as part of a team.
  • Proficient use of office equipment such as PC, laptop, tablet, Chromebook, fax machine, scanner, printer, and associated software applications. Ability to chart progress notes on EHR.
  • Excellent ability to communicate with clients and their families in a culturally sensitive and respectful manner and be empathetic and compassionate to the patient/client issues.
  • Able to prepare clear, coherent, and accurate activity and data reports.
  • Ability to sit and work at a computer for extended periods of time.
  • Must be able to lift and carry 40 lbs.
  • Must be highly self-directed and able to work independently toward obtaining goals for mandates.
  • Must have the ability to collaborate with community agencies for community partnerships with the purpose of establishing relationships and needs of patient/client.

Minimum Qualifications

  • Education
    • Master's Degree in Social Work or Marriage & Family Therapy
  • Experience
    • 4-6 years in the Social and Behavioral Health services
  • License
    • Hawai'i LCSW license required minimum of 1 year
  • Certifications and Other Requirements
    • Current TB clearance (2 step)
    • COVID-19 vaccination*
    • HIPAA Compliance
    • Bloodborne Pathogens
    • CPR/AED/First Aid Training

Desirable Qualifications

  • Familiar with HMONO's mission, programs, and services.
  • Possess a working knowledge of Native Hawaiian culture, traditions, history, and current events including, but not limited to State and Federally-funded programs and projects. Keep abreast with Native Hawaiian issues and current events.
