OVERVIEW:

Underthe leadership of the Registered Nurse, the LPN will be providing coordinatedcare to residents in a skilled nursing facility.

RESPONSIBILITIES:

• Providedirect care responsibilities to patients
• SuperviseCNA’s
• Administermedications and treatments timely and according to policy
• Receive,transcribe, and carry out physician orders
• Followestablished procedure for charting
• Completepatient assessments, coordination of care
• Identifyand reports changes in condition
• Followsenvironmental health & safety and sanitation standards and procedures
• Oversees clinical andnon-clinical acute operations and continuum of care

REQUIREMENTS:

• ValidLPN License in New York
• CPR/BLScertified
• 1 year nursing experience in a hospital, nursing home or other healthcaresetting
