POSITION OVERVIEW:Under the supervision of the Program Director for NY Connects, the Social Service Outreach Coordinator is responsible for providing all participants that call and/or visit, with free and objective information about long term services and supports (LTSS). The Coordinator must provide consistent, comprehensive, locally based information through the “No Wrong Door” system for individuals, caregivers, and families. The Coordinator must be willinging to work in the field and participate in outreach with various partnerships to build relationships and educate the community about NY Connects.
REQUIRED QUALIFICATIONS:Social Service Outreach Coordinator must have at minimum; a Bachelor’s degree in Social Work and/or related field, 1 to 2 years’ experience in the human and social service field, including service areas related to long term services and supports, case management and case assistance.The Coordinator must have proficiency in google suites and Microsoft office. He/She/They must also have experience in answering calls “call center” like environment, and providing assistance over the phone. Knowledge on benefits and entitlements. He/She/They must possess excellent customer service skills, attention to detail, communication skills and maintain the integrity of the organization at all times.
PREFERRED QUALIFICATIONSMaster’s Degree in Social Work; Strong organizational and communication skills, proficiency in computers. Bilingual in Spanish/English.
RESPONSIBILITIES:
WORKING CONDITIONS: Typical office environment that requires frequent sitting, standing, walking, talking, hearing & reaching. Occasional stooping, crouching, and kneeling. As well as travel to multiple locations within the five boroughs as needed.

PHYSICAL REQUIREMENTS: The position requires constant communication with participants and co-workers; must be able to exchange accurate information in these situations. Frequent operation of a computer and other office equipment, such as phone and copy machines. Ability to lift 25lbs and remain stationary for long periods.
