Huckleberry House is Hiring a Community Support Provider/Case Manager (Full-Time) Near Columbus, OH

Huckleberry House is now hiring a full-time Community Support Provider to provide case management and life skill training to youth ages 17-22 participating in our Transitional Living Program.

POSITION SUMMARY

Provides case advocacy and community support to young people and families aimed at meeting individual and family needs and increasing overall stability.

Provides independent living skill instruction to young people in housing program to assist them in successfully becoming independent & self-sufficient.

Participates in 24-hour on-call support.

  • Knowledge of independent living skills
  • Ability to assess client functioning and apply teaching methods appropriately.

MAJOR RESPONSIBILITIES / ACTIVITIES

  • Provides community support services aimed at meeting individual and family needs, and at the same time, helping to increase family stability.
  • Assists the client in developing a Housing Stabilization Plan and assists in implementing the plan for both mental health and independent living goals; actively evaluates plan progress and interventions to meet goals.
  • Provides referral support and linkage for youth with mental health issues and other applicable barriers to increase access to community resources and services.
  • Provides support and leadership to the Community Support Assistant (CSA) staff while assisting the team in carrying out the goals outlined in the Housing Stabilization Plan, which includes mental health and independent living goals.
  • Participates in 24-hour on-call support - by phone and/or in person as needed.
  • Provides day-to-day independent living and mental health support to clients, as well as crisis intervention as needed.
  • Attends weekly staff meetings, case review, individual and group supervision
  • Participates in service coordination for youth-serving teams with family and other providers.
  • Coaches youth on skills such as school enrollment, acquiring benefits through the Department of Job & Family Services (DJFS), assists youth with employment searches, and when necessary, accompanies and advocates for youth at their required court appearances, DJFS appointments, school meetings, etc.
  • Educates youth on the skill sets of budgeting, banking and money management, grocery planning and shopping for nutritional meal preparation, household maintenance skills, and options for accessing community healthcare, transportation and housing.
  • Administers life skill assessments, completes monthly information updates, collects rent, completes housing reviews and measures youth income, education, employment and housing status & plans for achievement.
  • Actively builds and maintains cooperative relationships with property owners to assist with housing stability upon program completion.
  • Plans and conducts group sessions with clients to address independent living skill development, mental health issues and to promote social skill development and community connections.
  • Transports clients in agency-owned or personal vehicle as needed.
  • Maintains required caseload and/or meets established productivity standards; maintains scheduling flexibility to accommodate client schedules and needs.
  • Accurately completes documentation in a timely manner.
  • Serves on an agency committee.
  • Completes other duties as assigned.

EDUCATION, CREDENTIALS AND QUALIFICATIONS

  • High School diploma or GED with postsecondary education preferred; related experience or training required with 3 years of experience required.
  • Strong verbal and written communication skills.
  • Ability to problem solve, resolve conflict, and negotiate.
  • Computer literate with strong data entry skills.
  • Ability to relate to and have empathy for young people.
  • Self-starter and works with minimal supervision.
  • Holds and maintains a valid driver’s license with a good driving record.

WORKING CONDITIONS AND PHYSICAL REQUIREMENTS:

  • Works at various locations in the community.
  • May require ability to sit, move about, climb stairs and operate telephone and personal computer for extended periods of time.
  • Must be accessible via cell phone.
  • Must furnish a reliable personal means of transportation in order to accomplish work objectives.
  • Must participate in on-call rotation, providing staff support outside of regular working hours.
  • Must be willing to offer evening appointment times two days per week.
