Hubbard Day and HH4K d/b/a The Foundry Center seeks a School Social Worker to join its multidisciplinary therapeutic and special education team. We serve developmentally disabled students who currently range in age from 5-21, most of whom have a diagnosis of autism. Our program is designed for students who require 1:1 instruction in order to gain skills and manage behaviors.

Job responsibilities include, but are not limited to:

• Providing 1:1 counseling for appropriate students
• Attending IEP meetings
• Developing IEP goals and objectives
• Attending classroom meetings and parent-teacher conferences as needed
• Collaborating to adapt and implement appropriate social thinking curriculum, social-emotional curriculum, and sex education curriculum for individual learners
• Collaborating to develop and implement appropriate social skills groups
• Coordinating parent trainings throughout the year
• Assisting families in navigating adult programs
• Facilitating transition planning
• Providing family-based supports, such as assisting parents in applying for OPWDD/Medicaid Waiver and maintaining a database of needed family resources
• Facilitating transition planning for transition age students (18-22 years old).
• Providing support and resources to families as part of the transition process, such as the Bureau of Rehabilitation Services ("BRS".)

Minimum Qualifications:

Masters degree in Social Work (MSW), #071 CT license, LMSW accepted

Preferred qualifications: LCSW, prior experience working in schools or with school-aged children, prior experience with developmental disabilities, prior experience facilitating transition services with families, familiarity with entitlement applications, previous experience in an educational field

Compensation: $67,500 per year or more, depending on license and experience level
