Summary: Home Health Social Worker Covering Springfield, IL and Decatur, IL Home Health/Hospice Territories Full Time Day Shift 40 hours/week Flexible Schedule LSW or LCSW required For more information contact Tara.Derosa@hshs.org Social workers play an essential role in many of the aspects of patient care, including helping patients and their families navigate the medical system, assessing and monitoring patients' and family members' mental and emotional health, providing short term counseling and therapy, and communicating patient needs and concerns to the larger medical team. Qualifications: Education Master’s Degree in Social Work is required. Experience 3 years of discharge planning is preferred. 3 years in social service field is preferred. Certifications, Licenses and Registrations Basic Life Support is required. Must be licensed in the state of practice.
