Our client, Pemi Baker Hospice & Home Health is seeking a motivated Licensed Practical Nurse to join our amazing Hospice and Palliative care team!
Pemi Baker Hospice & Home Health Mission is to provide the right care with kindness and compassion every day for every person we serve.
As a member of the interdisciplinary team, the LPN will provide nursing care to Hospice patients, which will include supportive care, physical care, pain management care and personal care to maximize the comfort and health of terminally ill patients and their families.
The LPN also provides nursing services to Home Health patients, contributing to the treatment and rehabilitation of the patient. Consults with and refers families to appropriate personnel within the Agency or other community services as needed.
