WHAT YOUR DAY ENTAILS

Embark on a rewarding journey as a Crisis Prevention Care Coordinator, playing a pivotal role in the lives of youth and families. Evaluate referral information, conduct assessments, and develop Crisis Prevention and Crisis Safety plans. Facilitate the creation of Child and Family Teams, engage with families through home/community visits, and maintain strong collaborations with service providers. Your role is instrumental in delivering strength-based, family-centered, culturally competent, and trauma-informed services!

WHAT WE NEED FROM YOU

• 21 years of age
• Bachelor's Degree in counseling and guidance, rehabilitation counseling, social work, education, psychology, or related human service field
• 2 years of supervising experience
• Knowledge of community programs, services, evidence-based practices, and various treatment modalities used to work with youth and their families to navigate child-serving systems
• Ability to actively participate in a fast-paced, multidisciplinary team environment
• Valid driver's license and proof of vehicle insurance
• Strong organizational and time management abilities
• Excellent interpersonal and written communication skills
