Houston Hospice is Hiring a Staff LVN - TMC In-Patient Unit Near Houston, TX

At Houston Hospice, we focus on caring, not curing. We take a team approach to care that includes physicians, nurses, social workers, chaplains, hospice aides, volunteers, and bereavement counselors. We provide hospice service wherever patients and families need us: in their home, in a long-term care facility, or in our inpatient care unit that is based in the Texas Medical Center. Our team of doctors and nurses specialize in managing pain and symptoms that are unique to life-limiting illnesses.

Qualifications:

  • Graduate of an accredited L.V.N. school or through reciprocity.
  • Currently licensed as an L.V.N. in the State of Texas.
  • One-year experience as an L.V.N.
  • Skilled in nursing practices with Home health, hospice, or oncology experience preferred.
  • Current certification in basic life support.
  • Ability to cope with family emotional stress and tolerant of individual lifestyles.
  • Good organizational skills required to handle a variety of tasks simultaneously.
  • Good communications skills, both oral and written.
  • Effective interpersonal skills.
  • Ability to accommodate multi-faith beliefs and backgrounds of patients/families.
  • Ability to function as part of interdisciplinary team.
  • Must have reliable transportation to work assignments.
  • Knowledge/proficiency in applicable Federal/State laws governing hospice licensure preferred.

Duties/Responsibilities:

  • Provide care for patients and families according to an individualized plan of care and in accordance with agency policies and regulations that define scope of practice.
  • Participate in shift assessments, administration of medications, and bedside care.
  • Assist patient/family in learning appropriate care techniques.
  • Assist and supports the R.N. and/or physician in performing specialized duties related to end of life care.
  • Report symptoms and changes in condition to the Charge Nurse, and physician.
  • Maintain accurate, up-to-date and timely documentation of nursing care provided to all patients.
  • Ensure problems, actions, specific outcomes and responses to care are clearly stated and readily available for review.
  • Rotate weekend/holiday duty as assigned.
  • Participate in QA/PI activities to assure continuous quality assurance and process improvement.
  • Assist in providing services that foster the organization’s mission, values and goals.
  • Responsible for timely completion of paperwork required to meet State and Federal regulations. Documentation should be accurate and detailed to meet these regulations.
  • Understand and adhere to organizational compliance and privacy programs.
  • Assists with orientation of new staff.
  • Performs other job related duties as required.
