Housecall Doctors Medical Group is Hiring a Lead Physician, Medical Director Near Los Angeles, CA

Homebase Medical is getting back to basics. Our mission is to optimize the quality of life for homebound older adults, allowing them to live independently and with dignity in their own homes. We make house calls – enabling access to quality wrap-around services for homebound older adults with complex health needs while improving the quality of their lives, right at home.

We offer an array of medical and chronic care services and we’re backed by SCAN, a not-for-profit organization that has a history of supporting and improving the health and independence of older adults for over 45 years.

The Lead Physician (Homebase Medical Director) will provide direct patient care across the spectrum of Homebase Medical’s lines of services and play a role in clinical leadership in partnership with the Homebase Medical CMO.

This is a mostly remote position that requires the incumbent to be in the Southern California area.

Responsibilities include:

  • Provide leadership and direction to physicians and clinical staff, promoting a culture of excellence and teamwork.
  • Develop and implement clinical policies and procedures to ensure compliance with state and federal regulations.
  • Collaborate with executive leadership to develop and execute strategic plans to achieve business goals.
  • Oversee the recruitment, onboarding, and retention of physicians and clinical staff.
  • Provide direct patient care to patients in their private residences, which may include, but is not limited to, private homes, apartments, senior living facilities, and nursing homes.
  • Serve as a liaison with other primary care physicians, hospitalists, ED physicians and specialists to coordinate care for post-acute care patients.
  • Prescribe medication in accordance with California statute and professional practice guidelines.
  • Provide clinical oversight and guidance to regional Advanced Practice Clinicians through documented collaborative practice agreements and in concordance with local regulations.
  • Provide direct feedback regarding clinical decision making and performance to assigned Advanced Practice Clinicians within assigned territory.
  • Work with the Health Information Management Team to ensure orders for Home Health and DME have been properly reviewed and signed by attending physician.
  • Collaborate with leadership to design and implement new workflows intended to enhance care delivery to members.
  • Be willing to provide education to clinicians related to geriatrics, complex care, and palliative care, usually by participating in the monthly clinician webinar.
  • Be available and provide support the clinician team in the field regarding urgent care needs for members such as change in medical condition or goals of care discussion.
  • Participate in community stakeholder meetings and address any issues related to care provided to members as needed.
  • Participate in on-call program to support team members telephonically after hours and/or weekends.
  • Participate in peer review, quality assurance, provider meetings, and other clinical meetings.
  • Actively support the achievement of Homebase’s Vision and Goals.
  • Other duties as assigned.

Qualifications and experience:

  • MD/DO, required
  • Internal Medicine Board Certification, required

  • Unrestricted medical and DEA license in California, required

  • Medical licensure in additional states is preferred (Pennsylvania, Arizona, Texas, Nevada)

  • 4 years of related experience as a physician, preferably with home-based visits, geriatrics, chronic disease management, and hospice and/or palliative care.
  • Demonstrated Geriatrics/Palliative care experience.
  • Experience with virtual care/telehealth preferred.

  • Demonstrated ability to lead and inspire a provider team.

  • Knowledge of the treatment of complex chronic diseases, such as dementia, diabetes, heart failure, and COPD.

  • Ability to identify team needs and coordinate with other resources/programs appropriately.

  • Knowledge of HCC and ICD-10 coding and documentation.

  • Knowledge of wound care and management.

  • Knowledge of CMS guidelines and Medicare Advantage Managed Care.

  • Comfortable with initiating goals of care and end of life discussions.

  • Strong interpersonal skills and change agent mindset needed to build relationships with healthcare team and patients.

  • Cultural competence and sensitivity required, multilingual skills preferred.

  • Knowledge of and ability to maintain HIPAA requirements.

  • Adaptable and open to change and problem-solving.

  • Knowledge of and/or ability to learn basic technological skills (Word, Excel, PowerPoint, Outlook, EMR) and open to new technology.
