SUMMARY: The Nurse Care Navigator is responsible for managing all intake processes required to efficiently process referrals for Palliative Care. services. This includes reviewing all clinical records, reviewing payer sources, obtaining all pre-authorizations, managing incoming calls, scheduling visits, assisting in the outpatient clinic, and providing education to patients and families on services within Hospice Savannah, INC when appropriate. The Nurse Care Navigator is committed to the philosophy, goals and objectives of the Hospice Savannah and Steward Center for Palliative Care concept while consistently demonstrating the Hospice Savannah values and compliance with organization’s services excellence standards.

QUALIFICATIONS: Graduate of an accredited school of Nursing, Licensed Practical Nurse license required with experience in hospice, oncology, physician offices, skilled nursing, assisted living or home health preferred. Knowledge of Joint Commission standards, state and federal regulations regarding care and reimbursement required with hospice specific knowledge preferred. CPR certification required. Previous experience in clinical education, preparing and planning educational in-services preferred. Valid driver's license, automobile Insurance and reliable transportation required. Excellent communication, customer service, leadership and critical thinking skills required to include conflict resolution and the ability to motivate others required. Computer proficiency in Microsoft Office and database knowledge required.
PHYSICAL DEMANDS: Position requires moderate to intense visual concentration, reading, using a computer, and reviewing patient charts. This position has daily contact with patients, families, medical facilities, physicians, physician office staff, vendors/suppliers and HSI staff requiring excellent communications, problem-solving skills, and professional demeanor. This position carries a risk of exposure to infectious diseases and requires skill in standard safety precautions. The Nurse Care Navigator requires the mobility to ambulate or lift a patient with or without the assistance of devices more than 75 pounds using proper body mechanics.
WORK ENVIRONMENT: This position is a 40-hour work week with occasional requests to work additional days/hours/holidays as needed. This position may require some commuting between the Hospice Savannah facilities utilizing a personal vehicle. This position encounters stressful situations with patients and their families regarding end-of-life care. The Nurse Care Navigator may be required to provide care in what has been determined by local authorities as a high-risk crime area and unsafe situations (potential or actual). The Nurse Care Navigator must be able to accurately, timely and concisely document the patient/family encounter using a computer.
