Summary of Duties and Responsibilities: Responsible for the delivery of social work services including initial psychosocial assessments, ongoing counseling, direct casework services, bereavement services, community education, outreach and referral

Position Type: Full-Time (32-40 hrs), Non-Exempt, Benefited

Job Details and Essential Functions:

  • Performs psychosocial assessments, ongoing reassessments, counseling/education, crisis intervention and referrals for patients/families as required
  • Documentation is completed in the patient’s electronic medical record (EMR)
  • Actively participates in coordination of all aspects of patient’s care, in accordance with current professional standards and practice
  • Participates/contributes in ongoing interdisciplinary comprehensive assessment
  • Develops and evaluates the Plan of Care (POC) and documents in the EMR
  • Serves as an advocate for the patient/family unit, and assists them with administrative, regulatory and financial procedures/paperwork including final arrangements
  • Serves on-call duties and maintains caseload as assigned by CSPM or the Director of Patient Care Services. Caseloads may vary based on management’s evaluation of patient/family and HOF needs
  • Participates in patient care meetings, and other Hospice meetings as requested
  • Participates in QAPI program and Hospice sponsored in-services
  • Participates in HOF orientation and in-service programs for staff and volunteers as requested
  • Performs other duties as assigned by the CSPM
  • May be assigned to any workplace setting based on HOF needs including office, RCFE, SNF and Home

Knowledge and Skills

Required

  • Ability to use independent judgement to make sound decisions
  • Maintain confidentiality and follow HIPAA and patient privacy laws
  • Strong, clerical skills including tying, copying, scanning and faxing
  • Critical thinking and provide necessary interventions
  • Communicate effectively, both orally and written
  • Public speaking, developing relationships with identified partners, groups, businesses with whom Hospice of the Foothills may have or need to establish affiliation

Education & Experience

Required

  • Two years’ experience in family and individual counseling and crisis intervention or home health social work

Desired

  • Two years’ experience in Hospice
  • Strong knowledge of state and Medicare regulations

License / Certifications

Required

  • Bachelors or Master’s Degree in Social Work
  • current CPR certificate
  • Valid California driver’s license, a reliable automobile and evidence of appropriate liability insurance

Pay: $63,128.00 - $77,584.00 per year
