Hospice of the East Bay is Hiring a Psychosocial Resource Specialist - LCSW Required Near Pleasant Hill, CA

Hospice of the East Bay (HEB) is a community-based hospice and palliative care program providing services in the home and at our six-bed Hospice facility for terminally ill patients and those in need of palliative care. Care is given to patients and families by an interdisciplinary group. Bereavement care is provided to the family for a minimum of one year after the death. Hospice and palliative care offer a sponsored care program to provide services regardless of patient’s ability to pay.

POSITION SUMMARY:

Reporting to the Quality and Education Manager this position is responsible for ensuring the competency of the HEB Social Work, Spiritual Care Counselors and Music Therapy Staff and for providing the supervision required for the MSWs who are preparing for licensure. This position is responsible for the education, support and evaluation of the Social Work, Spiritual Care and Music Therapy staff in coordination with IDT Managers and any other relevant sources. Other responsibilities include participating in the Quality Assessment and Performance Improvement Program (QAPI), conducting field observations and chart audits. Position inclusive of educating staff one-on-one when identified documentation or education needs are identified, recommending and assisting in the development of in-service instruction, when trends are identified, and assisting with preparation for and support during accreditation deemed status survey(s) and other surveys, such as Community Health Accreditation Partner (CHAP), Kaiser, California Department of Public Health (CDPH), and Department of Social Services (DSS) complaint surveys.

ESSENTIAL FUNCTIONS:

Clinical Staff Orientation and Competencies

  • Assures that new clinical hires receive appropriate clinical orientation, and coordinates such training with managers and Human Resources
  • Coordinates implementation and follow-up of competency requirements that are relevant to psychosocial and spiritual care assessments and goal interventions
  • Collaborates with IDT Managers if an employee is not competent or needs further orientation/supervision/development
  • Revises orientation program, as needed, in collaboration with the Manager of Quality and Education.

Clinical Staff Education

  • Uses information gathered from the QAPI Program and other relevant sources.
  • Monitors changes in California licensing requirements for LCSW status and for changes in industry standards for the provision of hospice psychosocial and spiritual care. Acts as a resource for student field placements, assuring contract in place, obtaining class objectives and course outline from school, working with Clinical Team Managers for assigned preceptor and schedule, monitoring performance and communicating with instructor, as assigned

In-Services

Quality Assessment and Performance Improvement

  • Participates as a member of the QAPI Committee and in Performance Improvement Projects (PIPs).

Safety and Infection Control

  • Participates in infection prevention and control activities and surveillance in the Home Program(s) and Bruns House, including monitoring the effectiveness of the IC program
  • Participates in response to Infection Control Outbreak

POSITION REQUIREMENTS:

  • Must have the ability to maintain confidentiality regarding patients and staff
  • Sensitivity to the various needs of different levels of staff and ability to teach the adult learner
  • Clinical knowledge of hospice and palliative care
  • Demonstrated ability to interface with an interdisciplinary team
  • Knowledge of the quality assessment and performance improvement (QAPI) process
  • Detail-oriented, with ability to communicate effectively with all staff members

EXPERIENCE & EDUCATION:

  • Current California licensure as LCSW
  • Experience designing and delivering educational programs and courses
  • Experience in cross disciplinary leadership
  • Computer skills required, including Word, Excel and PowerPoint
  • 3-5 years’ experience in hospice and/or palliative care
