Want to join our Team? We are currently looking for a Care Navigator Nurse Liaison (RN) (CNNL) to join our team. The CNNL develops and maintains professional relationships between Hospice of the Chesapeake and affiliates (HOC) and various hospitals in our supporting and service areas. Additionally, the CNNL will act as liaison between the hospitals and the hospice team to transition patients to the appropriate level of care, further increasing our overall census. The focus of these relationships is to drive growth in hospice utilization and our other service lines.

Office Location: Pasadena, MD

Coverage Area: Anne Arundel County

Status: Full-time – 40 hours/week

Salary: Competitive, commensurate with experience, education and credentialing

Main Responsibilities:

  • Drives growth in respective market.
  • Provides physical assessment to determine eligibility for hospice and appropriate level of care. Engages with patient and family to develop rapport and a sense of trust.
  • Maintains close relationships with healthcare professionals in assigned accounts/territory to maintain constant visibility of HOC programs and services and troubleshoots real or perceived problems that could impact patient referrals. 
  • Analyzes referral source patterns, determines reason(s) for the referral and develops targeted marketing plan(s) to maximize the further development of the particular referral source. 
  • If patient is to be admitted to services coordinates discharge with facility/hospital, establishes admission time with Access Department, and arranges for DME delivery. Communicates with SW or Discharge Planner to coordinate ambulance transport if necessary. For GIP eligible patients, arranges discharge to an HOC Inpatient Care Center, if patient is not in contracted hospital. 
  • Completes all electronic documentation in a timely manner, following appropriate protocol. Ensures that the documentation contains pertinent information regarding patient's condition, and appropriate focus of treatment. Uses appropriate agency forms, using approved abbreviations.
  • Represents HOC at professional/ clinical events. Prepares and makes presentations. 
  • Acts as a representative liaison between HOC and referral sources. Follows-up with concerns reported by facility staff and discussed issues with appropriate Clinical Team Managers or other appropriate managers.

Qualifications


Your Qualifications: 

  • Possess a current Washington DC nursing (RN) license and 2 years of med/surg or home health required. Hospice or Medical Sales experience preferred.

  • Dual Maryland RN license preferred by not required.

  • Demonstrated ability to provide quality patient care. Possess ability to assess patient’s needs, e.g. physical, teaching, mental and spiritual. Must have strong clinical skills.

  • Demonstrated knowledge and understanding of managed care organizations, including utilization review and capitated contracting concepts with hospitals and physician groups (physician billing, nursing home billing and hospice Medicare billing. 

  • Solid analytical, organizational, and interpersonal skills to effectively identify referral source problem areas, develop appropriate courses of action and convey same to client. 

  • Ability to perform consistently in stressful situations. 

  • Strong and creative, problem-solving skills with the ability to prioritize and delegate.

  • Demonstrated commitment to quality, timely, and cost-effective patient care.
