Full-Time Nights/12 Hour Shifts/On-Call/Rotating Weekends

7 on/7 off

Job Summary:

Primary functions are to triage and administer skilled nursing care for patients, of all ages, via phone or in person; coordinate care with the interdisciplinary team, patient/family, and referring agency; and assumes the responsibility for coordination of care.

Job Qualifications:

Education: Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing

Nursing Licensure: Current State license as a Registered Nurse, current Texas Driver's License

Experience: Two years experience as a Registered Nurse in a clinical care setting preferred, hospice or home health preferred.

Skills: Nursing skills as defined as generally accepted standards of practice. Good interpersonal skills. Specialized skills preferred in palliation and end-of-life.

Transportation: Reliable transportation. Valid and current auto liability insurance.

Environmental and Working Conditions:

Works in Homecare; possible exposure to blood and bodily fluids and infectious diseases; ability to work flexible schedule; ability to travel within the service area; some exposure to unpleasant weather; on-call; 12-hour shifts (rotating weekends)

Physical and Mental Effort:

Prolonged standing and walking required, with ability to lift up to 50 lbs. and move patients.

Essential Functions:

• Receive calls from Hospice patients and families, assesses the needs of the patient and family, and coordinates needed care over the phone, including scheduling home visits, as needed
• Assess hospice patients to identify the physical, psycho-social, and environmental needs
• Implement/develop/document the plan of care to ensure quality and continuity of care
• Provide clinical directions to the Hospice Aide/LVN to ensure quality and continuity of service provided
• Demonstrate commitment, professional growth and competency
• Promotes Agency philosophy and administrative policies
• Provide effective communication to patient/family, team members, and other health care professionals
• Provide home visits, as needed
