Job Description Summary:
The Nurse Practitioner (NP) is responsible for the coordination and delivery of hospice and palliative care services. The NP provides skilled nursing assessments, pain and symptom management, and applies interventions to maximize the comfort of patients and families. The Nurse Practitioner is a member of the IDT assisting in identifying patient/family needs and providing appropriate care during end-of-life.
Essential Job Functions/Responsibilities:
  • Works with the Chief Clinical Officer to deliver comprehensive, collaborative community-based hospice and palliative care services
  • Manages a Part-time Palliative care caseload; performs patient assessments and other medical procedures, including the prescription of medications consistent with the collaborative practice agreement and conforming to state and federal law at all times
  • Provides appropriate pain/symptom management, administers medicine within the scope of practice; evaluates patient’s response to treatment/medications
  • Collects, records, and analyzes assessment data such as patient health data, diagnoses, responses and problems, vital signs, imaging and lab results, etc.
  • Monitors and evaluates the patient’s responses, documents interventions, evaluates, and revises the plan of care as necessary.
  • Responds to patient’s medical or emotional crisis and/or death of a patient and makes home visits to provide direct patient care as needed
  • Completes Medicare-required face-to-face visits for hospice patients
  • Promotes the effective utilization of existing hospice and palliative care services as well as effective working relationships with other community providers to avoid duplication of services
  • Functions independently and/or as a member of an interdisciplinary team to plan, delivers, and evaluate health care for hospice and palliative care patients
  • Provides expert knowledge and modeling clinical skills for nursing staff
  • Participates in the development of patient/family education tools to meet identified care needs
  • Provides hospice and palliative care education to external agencies, community, and other health care providers; develops educational tools in response to identified needs
  • Provides education and serves as a resource for hospice nursing staff and other members of the interdisciplinary team as requested
  • Compiles and distributes educational material to patients.
  • Promote a mutually respectful atmosphere by actively supporting fellow staff members
  • Contacts the interdisciplinary team as necessary to report any identified needs
  • Consults, coordinates, and collaborates effectively with patients, family members, and other members of the health care team.
  • Effectively communicates with other staff members, management, and physician to assure continuous service to patients and timely resolution of issues.
  • Work collaboratively with the primary physician and members of the team, observing the patient for changes in condition, in order to meet positive patient care outcomes
  • Provide consultation, support, education, and recommendations for traditional and non-traditional medication orders
  • Reports, documents and communicates patient observations in EMR system; provides accurate and timely documentation of patient contact and services to reflect the Plan of Care
  • Supports patient satisfaction and customer service initiatives.
  • Adheres to HNCO payroll, billing and documentation policies and procedures
  • Comply with all policies and procedures
  • Participates in HNCO quality assessment performance improvement initiatives and activities
  • Participates in personal and professional development
  • Attends HNCO meetings, trainings, seminars and HNCO-sponsored events
  • Assumes responsibility for special projects as delegated by the Chief Clinical Officer or Medical Director(s)
  • Perform all duties and other reasonably related duties as assigned by the Chief Clinical Officer and Medical Director
Requirements:
Position Qualifications:
Experience and Education:
  • Master’s degree in Nursing required
  • One to three years of recent acute care, internal medicine/family practice, and/or palliative or hospice experience preferred
Required Certification/Licensure:
  • Current licensure to practice nursing in the state of Ohio
  • Current licensure as a Certified Nurse Practitioner in the State of Ohio
  • Current CPR certification
  • Valid Ohio driver’s license with current automobile insurance
  • Reliable transportation accessible for travel while on HNCO business
Management Experience:
  • None
Knowledge/Skills/Abilities:
  • Current nursing practices, principles, methods and procedures used to provide palliative and end-of-life care
  • Nursing assessment skills with/without visual inspection utilizing basic nursing theory
  • Good math skills; ability to complete dosage calculations accurately
  • Excellent written, verbal, and interpersonal communication skills
  • Good computer skills; EMR experience preferred
  • Excellent observation, critical thinking and problem-solving skills
  • Good judgment and decision-making based on palliative care and hospice guidelines
  • Assess data reflecting the patient’s status and interpret the appropriate information
  • Define problems, collect data, establish facts, and draw valid conclusions
  • Good organizational skills and ability to prioritize work
  • Assess and respond to the needs of patients and families in varied settings
  • Deal with people in crisis effectively
  • Work in a stressful environment and take appropriate action
  • Demonstrate sensitivity to issues of grief and loss; cope with family emotional stress and individual lifestyles
  • Empathize with the needs of ill, injured, frail and the impaired
  • Work independently and in a collaborative team environment
  • Maintain confidentiality of protected health information and general hospice information in accordance with applicable laws and HNCO policy
  • Demonstrate ethical behavior and HNCO core values
Physical Requirements:
  • Requires full range of motion, manual and finger dexterity and eye-hand coordination
  • Direct contact with patients, which may require moving patients, lifting, stretching, reaching, stooping, bending, kneeling and/or crouching
  • Prolonged or considerable walking or standing
  • Must be able to lift position and/or transfer patients, supplies and equipment
  • Visual acuity and hearing to perform required nursing skills
Continuing Education Requirements:
Organization personnel are expected to participate in appropriate continuing education as may be requested and/or required by their immediate supervisor. In addition, organization personnel are expected to accept personal responsibility for other educational activities to enhance job-related skills and abilities. All personnel must attend mandatory educational programs.
Environmental and Working Conditions:
Environmental Conditions:
May be exposed to extremes of heat and cold in all weather conditions. Must drive in various weather conditions on roads in varying degrees of repair.
Working Conditions:
May be exposed to infections and contagious diseases. Contact with patients under a wide variety of circumstances. May be exposed to/occasionally exposed to patient elements. Subject to varying and unpredictable situations. Handles emergency or crisis situations. Travel required.
OSHA exposure category:
Category I ? Position includes tasks that involve exposure to blood, body fluids and tissues.
Required Personal Protective Equipment:
mask, gloves, gowns, goggles (follows standard precautions using personal protective equipment as required for procedures)
