Hospice & Community Care of Rock Hill, SC is actively hiring an RN Case Manager to work Full Time!

Shift Details: Monday - Friday (8:00am - 4:30pm)

Hospice & Community Care is seeking an RN Case Manager to work Full Time to serve our community in and around the Rock Hill, South Carolina region. Our organization is fully dedicated to meeting the needs of patients and families, and helping them receive the care they need. As an RN Case Manager, you will be a part of a team that is committed to providing outstanding care and support to our patients and their caregivers. In this role, you will be an integral member of the Hospice team. Your success will be driven by your ability to lead and foster a culture of quality, compassion, and care.

What You’ll Do:

  • Assess hospice patients, coordinate and implement care plans, and evaluate interventions for hospice patients.
  • Lead hospice care team members in coordinating services for a small caseload of patients in accordance with the physician.
  • Provide outstanding nursing care, including teaching and supporting the patient as well as the caregiver. Care is provided through in-person visits and telehealth support.
  • Present services to potential patients, obtain initial physician orders, and complete all necessary paperwork related to nursing admissions and other visits as necessary.
  • Maintain necessary records to fulfill Federal and State legal requirements and policies.
  • Uphold operational standards including performance metrics, safety, and compliance.
  • Perform related duties and assignments as directed by management.

What You’ll Need:

  • Registered Nursing license in South Carolina, or be eligible for licensure in South Carolina.
  • Valid CPR certification
  • Team leadership skills
  • Experience in healthcare, home care, or hospice setting
