Want to join our Team? We are currently looking for a compassionate and dedicated Palliative Care RN Care Coordinator to join our team. As a RN Care Coordinator, you will provide a variety of nursing treatments including, but not limited to assessment of symptoms, patient/family education, and providing treatment recommendations within the nursing scope of practice. You will work with your team’s Social Workers, Chaplains, Nurse Practitioners and Physicians, to provide the physical, psychosocial, and spiritual aspects of care.

Office Location: Charles County Inpatient Care Center- 2505 Davis Road, Waldorf, MD 206030

Coverage Area: Charles and Prince George’s Counties. Occasionally, Calvert and Anne Arundel Counties

Status: Full-time

Schedule: Monday – Friday 8am – 4:30pm

Salary: Competitive, commensurate with experience, education and credentialing

Main Responsibilities:

  • Provides appropriate, accurate and timely information in response to clinical calls and inquiries to patient/families. Determines via telephone, callers issues and needs; effectively analyzes their implications and takes appropriate and timely action and/or recommendation. 
  • Answers phones promptly and uses knowledge to describe the palliative programs and the benefits provided to the patients.
  • Collects data for all referrals to include but not limited to, patient demographics, diagnosis and insurance. Maintains an ongoing spreadsheet for easy retrieval of patient information.
  • Triages all appropriate urgent calls to the physician/nurse practitioner and leaves timely messages regarding non-urgent calls
  • Other specific goals and duties as communicated in your job description and performance goals.

Qualifications


Your Qualifications:

  • Current Maryland RN license or accepted reciprocal out-of-state nursing licensure; BSN preferred.
  • 2 or more years of nursing experience in med/surg, long-term care, or home care required, with experience in hospice, end-of-life, or critical care experience preferred.
  • Familiarity and comfort with documenting in an Electronic Medical Record.
  • Valid driver’s license with an acceptable driving record and reliable transportation
  • Ability to meet the physical demands of the position with or without reasonable accommodation.
