Clinical Social Worker, Palliative Care

Primary Function
Responsible for overseeing and managing the social work care required to meet the needs of patients and caregivers. To provide clinical social work services for patients and their caregivers by assisting with psychosocial issues. Provide psychotherapy as indicated for patients and patient families.
Qualifications
1. Master’s degree in social work from an accredited school in social work required.
2. South Carolina clinical social work license required. (LISW-CP)
3. Social work experience in medical field preferred.
4. Ability to work well within an interdisciplinary group setting as well as independently.
5. Ability to work a flexible schedule.
6. Experience with mental health insurance billing preferred.
Physical Demands
Work requires the physical demands of frequent sitting, written and verbal communications; occasional bending/squatting, reaching above shoulder level, crouching/kneeling, opening, and closing file drawers, lifting/carrying and fine manipulation when working with computer and telephone. Essential sensory requirements are visual acuity, touch, and hearing.

Job Responsibilities:

a). Coordinates care to palliative patients including comprehensive admission assessment; psychosocial assessments; counseling; development and implementation of the plan of care related to psychosocial needs.
b) Responsible for completing documentation as per policy. Ensures the clinical records are maintained in compliance with state and federal guidelines.
c)Perform initial and ongoing assessments of patients and caregivers and documentation as required.
d) Provides counseling/therapy and education to patients and caregivers.
e) Assist with financial concerns of patient and caregiver and make appropriate community referrals as indicated.
f) Provide assistance to the patient and caregiver in the utilization of all appropriate community resources as indicated.
g) Participate as a member of the Interdisciplinary Group
h) Establish and implement goals in the patient care plan to meet the psychosocial needs of the patient and caregiver.
i) Participate in Performance Improvement studies as indicated.
j) Attend in-services and continuing education programs to keep current on regulations. Completes a minimum of twelve hours hospice related in-services per year.
k) Document in chart all visits and/or phone calls to patient/caregiver and report to staff.
l) Complete social work portion of patient/caregiver discharge summaries, re-certifications, etc.
m) Work with staff, local agencies and community groups as opportunity arises to enhance the psychosocial concerns of patients and caregivers experiencing terminal illness and loss. May include marketing or speaking engagements.
n) Participate in agency surveys.
o) Perform other duties as assigned by supervisor.
