FT 8am-4pm. (1) weekend every (4) weeks.

A RN Hospital Liaison is responsible for presentation of Hospice services to clients who have been referred for all levels of care (general inpatient, home care, and nursing home). Facilitates and completes the admission process after reviewing patient/family goals of care and identifying the appropriate level of care. Initiates individual professional IDT plan of care. Collaborates with the IDT, physician, and facility staff to determine start of care orders. Provides in-services for hospital staff and the community.

Essential Duties and Responsibilities for the RN Hospital Liaison:

  • Presentation of hospice services to patients and families is individualized, sensitive to patient and family's fears, needs and goals, demonstrates a knowledge of guidelines for hospice eligibility (LCD, CCMR's) and related HPCB programs, and appropriately identifies the level of hospice care.
  • Demonstrates ability to perform comprehensive assessments to identify all physical, psychosocial, environmental, safety, emotional and spiritual needs related to the terminal illness that must be addressed in order to promote the patient's well-being, comfort, and dignity throughout the dying process and documents findings appropriately in the EMR.
  • Performs pain assessments using, adhering to pain protocols, knowledgeable in equi-analgesic dosing; optimal pain relief using an interdisciplinary focus and timely interventions.
  • Able to articulate clinical course/disease progression of cancer(s), lung disease, heart disease, liver disease, dementia, neurologic disease states.
  • Able to articulate etiologies of symptom manifestation (nausea, vomiting, anorexia, fluid and electrolyte imbalance, diarrhea, constipation, mucositis, weakness/fatigue, dysphasia, edema, dyspnea, cognitive impairment, urinary retention, depression, delirium) related to a disease process.
  • Initiates the plan of care timely and appropriately with a demonstrated skill in anticipating problems that may cause or create a threat to patient safety or lack of symptom control.
  • Demonstrates knowledge in usage, indication, adverse side effects, contraindications and negative interactions of most commonly used steroids, NSAIDs, benzodiazepines, antidepressants, anticonvulsants, narcotics, antifungals, diuretics.
  • Demonstrates proper sharps disposal, hand hygiene, waste disposal and personal protective equipment.
  • Displays a cost conscious orientation towards care delivery, providing effective and cost efficient care.
  • Organizes consistently to meet productivity standards, incorporates computer documentation into patient visit and completes documentation requirements within time parameters.
  • Executes patient/family/caregiver teaching in all given situations.
  • Adheres to requirements of both HPCB and Hospice Buffalo policies and procedures that are both patient focused (clinical) and organizational focused (administrative). Adheres to HIPPA regulations and policies. Maintains the security of patient electronic/written records.
  • Elicited no founded patient/family complaints or unusual occurrences (i.e. reportable incidents).
  • Displays an ability to establish relationships and maintain/collaborate positive rapport with other access and hospice clinical staff and hospice support staff, hospital staff, and the medical community.
  • Demonstrates a bias for action, acts on what is important, is proactive and creative, possess a "can do" attitude, has an ability to work under pressure and keep difficult situations in prospective.
  • Participates in the weekend, holiday schedule.

Qualifications for a RN Hospital Liaison:

  • Education/Certification
  • Graduate of an accredited school of nursing with current RN licensure in the State of New York.
  • Certification in Hospice and Palliative Care Nursing preferred.
  • Valid Driver's License, free of serious violations, and vehicle required for use when on duty or on-call.
  • Experience
  • 2 years of nursing practice as a professional nurse is preferred.
