Wound Ostomy RN Position Summary: Under direction of the Executive Director of Quality and Survey Excellence, the Wound Ostomy Registered Nurse position ("WORN") is responsible for developing and implementing a comprehensive wound care program across the organization’s various lines of patient care (hospice, palliative, and PACE). The WORN must demonstrate excellence in nursing practice for all assignments and be supportive of the organization’s needs in providing nursing care and services to all patients in all business lines. Position Duties & Responsibilities: Establishes organizational standards of care, competencies, policies, and procedures for quality, cost efficient and effective wound care.Serves as a consultant to all organizational patient business lines (hospice, palliative care, and PACE) for wound care recommendations, as identified and needed.Provides consultative wound care services and recommendations to all staff, including for individualized patient plans of care.Manages routine quality audits on the organization’s wound care documentation and practices, and reporting those results.Reviews and updates the organization’s wound care policies and procedures, using industry standards of care and evidence-based support.Assists with the development of wound care performance improvement plans and related education (in coordination with applicable staff, including but not limited to the Clinical and Education departments).In consultation with physicians, assists the primary nurse or nurse manager in developing an appropriate plan of care for wound management in relation to patient disease process, when needed.Makes recommendations for all aspects of patient wound care and provides evidence-based research and support.As part of the organization’s quality assurance program, develops, audits, and reports on the organization’s wound care, including chart documentation and in person observations (as assigned). Participates in the organization’s quality outcome monitoring, follow-up, performance improvement initiatives, and education to staff.Monitors the organization’s electronic reporting system on wound care and investigates adverse patient wound care events.Maintains a cost-effective wound care formulary.Upon occasion, and primarily with other field staff for educational purposes, conducts on site patient wound assessments and care related to complex or high-risk wounds, and appropriately documents same to include all interventions performed and care goals.Collaborates, coordinates, and communicates with staff at all levels in all organizational business lines related to wound care practices, needs, supplies, and recommendations for improvement to provide high quality patient wound care.Helps develop wound care education for staff addressing proper wound protocols and care and wound prevention and assists with the delivery of same, as needed and assigned.Stays abreast of the latest advancements in wound care treatments and research, integrating new evidence into the organization’s practices.Participates in organization quality-related committees and meetings, as requested or assigned. Assists with the organization’s preparations for external survey(s).Other duties as assigned. Position Qualifications: RN with current RN licensure in the state of Kentucky and Indiana. BSN preferred. Certification in Wound, Ostomy, and Continence Nursing (WOCN) or Certified Wound Care Nurse (WCN) plus one year of wound ostomy nursing required. Proficiency in wound assessment techniques, wound care procedures, and evidence-based practices.Certification in hospice/palliative care preferred.Must have an ability to travel to locations within the organization’s footprint.Proficient technology skills, including Microsoft Office Suite, Excel, and EMR.As a member of the Legal Department and Quality Team, ability to maintain strict confidentiality of organization matters related to attorney-client, compliance, quality, and risk.
