Hosparus Health is Hiring a Community Health Worker Near Louisville, KY

Must have Community Health Worker certification or be in an active apprenticeship working toward the certification. Position Summary: The Community Health Worker serves as a contact and resource for securing resources for patients and families. This includes assisting in completing requests from the Patient Family Support Fund as they are approved. The Social Services Liaison will also work directly with social workers in assisting to navigate the Medicaid application process and processing the paperwork. The Social Services Liaison may also work directly with Nursing Care Facilities (NCF) to assist in developing relationships and collaborating between the social workers, patients and families, and the facilities by conducting pre-screening for identified potential patients in need of Medicaid services and assisting with respite bed placement. Position Duties & Responsibilities: Identifies patient and/or family needs and connects patients with culturally-competent resources within the community. Serves as a resource to connect patients and families with community resources, such as food pantries or utility assistance. Establishes relationships with new and changing community resources for patients. Coordinates referrals to community partners, when appropriate, including procuring all required consents.Communicates regularly with clinical team on patient needs; serves as a liaison between patient and clinical providers when necessary.Assists with the completion of patient or family requested documents, including FMLA certifications, letter requests from families, and handicapped parking applications. Obtains signatures of needed forms such as Change of Attending, ROI, MAP forms, etc.Coordinates respite and long-term care placement needs with families and social work teams.Assists with key logistics of the Medicaid application process, including delivery of the initial packet to obtain signatures and back for application.Delivers supplies or other items from the IDG team or volunteer department as needed.Assists with Patient Care Fund Requests in collaboration with IDG team, patient or family, and or Social Work Liaison. Gathers information on cremation options, VA burial benefits, or indigent cases and provides family with resource information at the direction of the team social worker.Documents all patient interactions in accordance with this position’s standard processes.Attends and participates in departmental and hospital-wide meetings.Other duties as assigned. Position Qualifications: Must have Community Health Worker certification or be in an active apprenticeship working toward the certification. Must have ability to communicate effectively with professionals of various disciplines, as well as with clients. Must be non-judgmental, culturally competent, and able to work with clients of differing social, economic, political, or religious lifestyles or beliefs. Excellent computer skills required, including Word and Excel.Able to maintain strict privacy and patient confidentiality as well as appropriate patient boundaries. Excellent organizational and problem-solving skills required.Ability to assume responsibility and work independently with the flexibility to adapt quickly to changing situations.
