JOB TITLE: Case Manager

LOCATION: Cronin House -Hayward, CA

How can you make a difference in people’s lives?

In this role, you’ll make a difference in the lives of people with substance abuse challenges, and often with co-occurring conditions. You’ll use and grow your skills in compassionate care, crisis intervention and de-escalation, in a supportive and inclusive work environment. You’ll have the opportunity provide our community with substance abuse treatment to help people who are just discovering possibilities in their journey, going from a place of hopelessness to a place where they are valued and have the potential for a better life.

The Case Manager is responsible for assessing treatment needs; developing, monitoring, and evaluating treatment plans and progress; and facilitating interdisciplinary approaches. The Case Manager coordinates an approach to the delivery of health, substance use, mental health, and social services, linking clients with appropriate services to address specific needs and achieve stated goals. The Case Manager duties include care coordination, maintaining positive working relationships with outside community partners, scheduling appointments, arranging transportation, assisting with acquiring medications, ongoing care and discharge planning, and crisis intervention. The Case Manager will collaborate frequently with Intake Counselors and report to the LPHA to ensure clients basic needs are being met in preparation for stabilization and effective discharge planning.

Responsibilities:

Provide clients a single point of contact for support addressing multiple health, social services and legal systems

• Advocate for clients engaging in services within the community

• Create a safe space for clients and maintain appropriate boundaries

• Be flexible, community-based, and client-oriented

• Assist the clients with needs of substance use and mental health as well as assistance thought to be outside the realm of substance use and mental health treatment

• Able to learn and apply various models and theories of addiction and mental health and other problems related to substance use

• Ability to describe and practice the philosophies, practices, policies, and outcomes of the most generally accepted and scientifically supported models of treatment, recovery, relapse prevention, and continuing case management for addiction, mental health, and other related problems

• Organize client transport to their appointments

• Transport clients to appointments when necessary

• Provide support in Milieu when needed

• Crisis management and de-escalation

What will make you successful in this role?

• Ability to use a computer; MS Word, Excel, PowerPoint, database systems, write reports

• Knowledge of demographic data collection

• Safeguard Protected Health Information (PHI)

• Ability to manage time and prioritize

• Experience presenting programs and services in the community.

• Ability to execute projects and measure results/ impact

• Experience working with customers or clients that feel overwhelmed or in a crisis (crisis intervention)

• Experienced working with diverse populations and cultures

• Able to exercise good judgment and maintain healthy boundaries with participants.

• Able to drive a 10-passenger van.

Measures clients progress and treatment outcomes. Recognizes and rapidly addresses errors in care and collaborates with other team members on service improvement.

What else is required?

  • Registration or Certification with a Substance Use state certifying body (CAADE/CCAPP/CADTP) in California
  • Valid California driver’s license with a good driving record and car insurance.
  • Current CPR/First Aid certificate desirable.
  • Background Check/drug testing required at time of hire.
  • Current flu vaccine/flu shot.
  • The California Department of Public Health has required that all staff in healthcare organization receive the COVID 19 and boosted. Our organization is a healthcare organization and will abide by this mandate.

Salary: $27.00-$28.60 an hour based on experience and qualifications. Available for a full-time position.
